The medical crews were unable to free the driver in time to provide medical help to the woman.
The train driver died after two trains collided on the Riverside Line in Newton, near Boston, just hours after an elevated train derailed in Chicago.
One train was stopped at a red signal and was ready to roll to the station when it was hit from behind.
Seven passengers were taken to local hospitals, and one was airlifted to a Boston hospital, officials said. About 10 passengers were injured.
The medical crews were unable to free the driver in time to provide medical help to the woman, identified as 24-year-old Terrese Edmonds.
